The 2026 proposed CPT code updates from CMS are expanding reimbursement for Remote Patient Monitoring (RPM), creating new opportunities for practices and RPM solution providers. If approved, these new opportunities will be in effect January 1, 2026!

What’s Proposed to Change in 2026

The proposed updates expand reimbursable RPM services and reduce barriers:

  • 2–15 days of readings now count toward reimbursement (in addition to the current code for 16+ days)
  • 10 minutes of clinician review time (in addition to the current 20 minutes) qualifies for billing
  • More flexibility to align reimbursement opportunities with your care delivery approach and patient care plans

These proposed changes make RPM more accessible, efficient, and financially viable for practices and partners.

RPM Reimbursement 101

RPM allows providers to monitor patient data remotely — including blood pressure, glucose, weight, and oxygen levels — using FDA-cleared devices that automatically transmit patient data.

Current CPT codes:

  • 99453: Initial setup and patient education
  • 99454: 16+ days of device use and data transmission every 30 days
  • 99457: First 20 minutes of clinical review and management
  • 99458: Each additional 20 minutes of clinical review and management

Currently, providers must capture 16+ days of readings in a 30-day period to qualify for reimbursement. Similarly, providers must complete 20 minutes of review time for reimbursement. Under the 2026 proposal, both requirements are eased — allowing reimbursement for as few as 2–15 days of readings and just 10 minutes of review.
