What’s New in 2026
CMS has adopted the AMA’s recommendations to:
- Reduce the minimum device reading days for reimbursement: 2–15 days now qualify (in addition to the existing 16+ days of readings code)
- Shorten required clinician review time: 10 minutes of interactive review qualifies for billing (in addition to the existing 20 minutes)
- Increase flexibility to better align reimbursement with your care delivery model and patient care plans

RPM Reimbursement 101 + Important Details on New 2026 Codes
RPM allows clinicians to remotely monitor patient data, including blood pressure, glucose, weight, and oxygen levels, using FDA-cleared devices that automatically transmit patient readings.
New CPT Codes for 2026:
- 99445 – Device supply, 2–15 days of readings
- 99470 – First 10 minutes of clinical monitoring time
Existing CPT Codes:
- 99453 – Initial setup & patient education
- 99454 – Device supply, 16+ days of readings
- 99457 – First 20 minutes of clinical review
- 99458 – Each additional 20 minutes of clinical review
New 2026 CPT Code Changes and Usage
Currently, 99453 requires 16 days of readings in order to submit for reimbursement. Beginning January 1st, it will reflect the new code changes and will only require 2 days of readings to be billed. This clarification ensures providers have clear guidance and can confidently bill 99453 for even short-term device use and patient education.
In addition, CMS has clarified how providers should select the appropriate RPM code based on days of device readings and minutes of clinical monitoring time:
- 99445 vs. 99454 (Days of Readings):
- Use 99445 when a patient records 2–15 days of readings within a 30-day period.
- Use 99454 when a patient records 16 or more days of readings within the same 30-day period.
- 99470 vs. 99457 (Clinical Monitoring Time):
- Use 99470 for 10–19 minutes of clinical monitoring and interactive communication per month.
- Use 99457 when clinical monitoring time meets or exceeds 20 minutes per month.
- 99458 (Additional Monitoring Time):
- Use 99458 for each additional 20 minutes of clinical monitoring beyond the initial time billed under 99470 or 99457.
